Enrolling Your Child

Seba Dalkai Boarding School serves students in kindergarten through Grade 8. Families interested in enrolling their child must complete an enrollment application and submit the required documents.

How to Enroll

1. Request an enrollment application. Contact the school office for the current application and enrollment instructions.
2. Complete the application. Fill out all required forms and provide the requested information.
3. Gather supporting documents. Review the list below to help prepare for enrollment.
4. Submit your paperwork. Follow the school's instructions for returning the completed application and supporting documents.

Enrollment Documents

Families should be prepared to provide the following documents when enrolling their child:

• Birth certificate: A certified copy of the student's birth certificate or other accepted proof of age.
• Tribal verification: A Certificate of Indian Blood (CIB), tribal enrollment documentation, or other accepted documentation of tribal eligibility.
• Immunization records: The student's current immunization records or applicable exemption documentation.

The school may also request additional information, such as:

• Parent or guardian identification.
• The student's most recent report card or school records.
• Custody or guardianship documents, if applicable.
• A current Individualized Education Program (IEP) or Section 504 plan, if available, to help coordinate student support services.
